Training on Assistive Technology (AT) in Transition Planning

Dear All,

Please see information below about an upcoming training opportunity.

Who is this training for?
If you provide services to individuals transitioning into the community from an institutional setting then this training is for you! Case managers, independent living (IL) skills specialists/trainers and others who assist people in transitioning to the community are strongly encouraged to attend.

What is the TACLE AT assessment tool?
This simple-to-use assessment tool is designed for case managers, social workers, healthcare providers and related agency workers to use to determine the need for assistive technology services and possible referral for individuals transitioning out of institutions. The tool provides a supplement with examples of AT in areas such as Safety and Mobility, Personal Care, or Communication for folks who work with consumers to transition from institutional living into the community. It will be much appreciated if you will disseminate the information through your networks/providers.
